The presence of a duplicated urinary bladder and prostate has not been described. We report the case of a complete duplication of the urinary bladder and prostate, associated with one urethral meatus and a bifid urethra.

A 68-year-old male presented with chronic lower urinary tract symptoms (LUTS), recurrent urinary tract infections, bladder stone removal and epispadias repair. Bladder duplication was incidentally found during imaging. Cystoscopy confirmed one meatus with two bladders and urethras, each with its own prostate. Transurethral resection of both prostates (TURP) was performed.

This is the first reported case of complete duplication of both bladder and prostate.
