Vaping and orthodontics: addressing the emerging risks

With vaping rates continuing to rise among adolescents and young adults, dental professionals are increasingly being asked about its impact on oral health. At this year's Dentistry Show London, Specialist Orthodontist Joseph Bell explored this important issue in his session: ‘Vaping, orthodontics and oral health: understanding the evidence, identifying the risks, and supporting patients'.

Bell, a specialist in orthodontics undertaking consultant training, explained that his interest in the subject began after seeing one of his own brace-wearing patients vaping. ‘We already ask our patients to avoid fizzy drinks and maintain strict hygiene. Vaping adds another layer of complexity,' he reflected.

While the evidence base remains limited – with most data derived from in vitro studies or animal models – Bell highlighted early findings that suggest vaping may increase plaque retention, reduce salivary flow, and negatively affect the performance of orthodontic materials. ‘There's emerging evidence that vapour exposure may weaken elastomeric chains, potentially slowing treatment.'

In response, his department has updated medical history forms to include vaping status and undertaken a quality improvement project to strengthen staff education. ‘Asking the question directly increased disclosure, especially in over-18s.'

The wider concern, he emphasised, lies in behavioural trends. ‘Many young people who vape have never smoked. That's where the risk really is. Clinicians need to support informed choices and promote prevention.'

Bell encouraged dental teams to adopt a proactive but non-judgemental approach – initiating conversations, recognising potential signs, and supporting patients in behaviour change. He reminded delegates that orthodontists in particular see patients every 6–8 weeks, placing them in a unique position to provide ongoing education.

Joseph Bell's presentation was delivered on 3 October in the Specialty Interest Theatre, and it contributed to a wider programme at Dentistry Show London focused on emerging challenges and opportunities for the dental profession.
